Monsters refers to the many antagonistic creatures the player will encounter throughout the game.
There are approximately 84 naturally spawning entities in the IPECAC Community Mod currently, with some extras that are unfinished or will not spawn without the use of commands.

Amygdala | Skitters aimlessly. After being damaged, runs in a vague circle around the player, leaving red creep and sometimes shooting 3 shots. After a while, returns to walking aimlessly. |
Aneurysm | Moves around the room diagonally, occasionally firing brimstone in an x or + pattern, shown by a red glow around it. |
Appendix | Moves around the room diagonally, leaving shots in where it goes. Moves faster on later floors. |
Bab | Generally found in sets of three orbiting a Momma. Occasionally fire a tear at Isaac. At 50% HP, they will detach and start chasing Isaac. |
Bivalve | The masks are invulnerable, and chained to the heart. After a while, each of the masks breaks free. Once the heart dies, both masks die. |
Bladder | Stationary enemy that decreases in size when hit, but regenerates to its full size until fully destroyed. When it is at full size, it shrinks again and creates a line of damaging yellow creep towards Isaac. |
Blind Eye | Slowly hops towards Isaac, leaving slippery blue creep in its wake. |
Bloatshroom | Slowly walks towards Isaac. Occasionally releases a large spore cloud-like volley of shots that stay in place, slowly falling to the ground. Releases another "spore cloud" on death. |
Blud | Invulnerable enemy that chases Isaac, creating a damaging trail of creep wherever it goes. |
Brown Knight | Moves erratically around the room and charges toward Isaac when in line of sight, can only be damaged from behind. and flies over obstacles, leaving a trail of slippery creep. When damaged, sometimes spawns brownie corns or shoot corn projectiles from its back. |
Brownlet | Crawls around the room erratically, following Isaac and dealing touch damage. Can jump small distances. Occasionally stops and spits three corn shots at Isaac, or spawns a corn mine. It can also shoot at Isaac while jumping. |
Caster | Wanders around the room and occasionally spawns a purple fire. Only two purple fire can exist at a time. Also spawns a purple fire on death. |
Cateratic | Walks towards Isaac, dealing contact damage. At 66% and 33% health, one of its eyes will pop out, and become a Blind Eye. |
Chaos Wisp | Groups up with other wisps in the room, and circles a random enemy, protecting it from shots. Speeds up all other wisps in the room, and occasionally fires a homing tear at Isaac. |
Conjoined Maw | Slowly moves towards Isaac, occasionally stopping to shoot a ring of shots in random directions. When killed, spawns 1-2 Maws |
Corn Squirt | Dashes towards the player, leaving a trail of slippery creep and occasionally shooting and deals contact damage. Spawns 2 Brownie Corns upon death. |
Cringa | Slowly moves around the room, occasionally hopping, spawning Corns or Brownie Corns, firing 8 corn shots in all directions and spawning slippery creep. Spawns 2 Corn Squirts on death. |
Crosseyed Stone Shooter | Stationary and invulnerable monster that continuously fires shots in either 45° left or right of the direction it's facing. |
Cursed Cyclopia | Walks towards Isaac, dealing contact damage. When it has a direct line of sight of Isaac, will fire a quick burst of four tears in his direction. |
Cyclobin | Chases Isaac, dealing a full heart of contact damage. Occasionally stops for a brief while and shoots a big projectile which splits in 4 on an x pattern when it hits the ground. Upon taking enough damage, it collapses into a pile of goo, which, if not killed, will regenerate. |
Demon Host | Stationary enemy that is normally invulnerable. Raises its head up when Isaac gets close, revealing a fleshy stem that can be damaged. Upon raising its head, it will fire a brimstone laser at Isaac and lower its head again. |
Dinkie | Shuffles towards Isaac, dealing contact damage. Occasionally stops to shoot a single poo shot at Isaac. Splits into two Drops on death. |
Dookie | Shuffles around the room in random directions. Occasionally stops to shoot two poo shots at Isaac in a V formation. Splits into two Dinkies on death. |
Door Mimic | Stationary and invulnerable enemy that randomly spawns in the position a door could when Isaac first enters a room. Will punch Isaac if he walks into it. |
Double Creep | Crawls along walls to try to line up with Isaac and shoots six blood shots in a double-helix pattern. |
Double Worm | Pops out of the ground in random locations, revealing either its head or tail. The head will shoot a small spray of shots at Isaac. The tail shoots a small number of explosive shots in random directions. Only the tail can be damaged. |
Dozer | Stationary enemy that is harmless, so long as it is not shot or approached. If Isaac shoots it, gets too close, or makes a loud noise (such as causing an explosion) it will wake up, and begin charging randomly around the room, changing direction when it hits a wall. |
Dr. Grimace | Stationary and invulnerable monster that periodically spawns crosshairs around the room, whereupon a Epic Fetus missile will land shortly after. Stops firing when room is cleared. |
Drooler | Alternates between fleeing from the player and chasing them |
Drop | Quickly shuffles towards Isaac, dealing contact damage. Dies if it touches him. |
Dropdown Flyder | Stationary and invulnerable monster that will drop down from the ceiling if Isaac gets too close, becoming a Flyder. |
Dropdown Spider | Stationary and invulnerable monster that will drop down from the ceiling if Isaac gets too close, becoming a Spider. |
Epic Grimace | Stationary and invulnerable monster that occasionally spawns a crosshair beneath Isaac, whereupon a Epic Fetus missile will land shortly after. Stops firing when room is cleared. |
Flaming Horf | Stationary enemy that fires a long-range fire projectile at Isaac when he gets in range of them. |
Fleshhead | When standing, makes projectiles close to the player that are moving perpendicular to it turn 90 and face the player. Also deflects tears going in its direction. After a while, retreats and becomes vulnerable. |
Flite | Flies erratically around the room, loosely in the same direction as Isaac. Occasionally stops to charge up a ramming attack, launching itself in Isaacs direction, only stopping when it hits a wall. |
Floating Brown Knight | Flies erratically around the room and charges toward Isaac when in line of sight, can only be damaged from behind. and flies over obstacles, leaving a trail of slippery creep. When damaged, sometimes spawns brownie corns or shoot corn projectiles from its back. |
Fly Host | Stationary enemy that is normally invulnerable. Raises its head up when Isaac gets close, revealing a fleshy stem that can be damaged. Upon raising its head, it spawns an attack fly and lowers its head again. |
Flyder | Erratically moves towards Isaac and deals contact damage. Occasionally takes flight and slowly moves towards him, flying over obstacles. |
Frail Worm | Pops out of the ground in random locations and spits a stream of blood shots at Isaac if he is in sight. |
Glitches | Walks towards Isaac, dealing contact damage. Teleports around the room at random intervals, or if it takes damage. |
Hematochezia | Dashes towards the player, leaving a trail of red damaging creep, before quickly stopping. If it hits any enemy, obstacle, player or bomb, it immediately stops, shooting two volleys of projectiles - one towards its movement and one against. Instead of dying, becomes a mound and regenerates after some seconds. Dies upon room clear. |
Infected | Walks towards Isaac, occasionally stopping to fire a poisonous gas cloud at him. If any gaper-like enemies walk into this cloud, they will also become an Infected. |
Leecher | Aimlessly hops around the room, until Isaac gets close, where it will try and flee, shooting projectiles and creating red creep in the process when it lands. Deals contact damage and recovers health whenever it damages someone. |
Lovely Grimace | Stationary monster that periodically tries to heal all enemies in the room. Unlike all other grimaces, the Lovely Grimace will temporarily disable when it receives enough damage. |
lvl2 Flyder | Crawls around the room erratically, following Isaac and dealing touch damage. Can jump small distances. Occasionally takes flight and tries to fly towards Isaac. |
Minodulo | Hops towards Isaac, occasionally spitting out an arcing shot that leaves creep where it lands. Occasionally jumps high into the air, dropping down onto Isaac's position while firing 4 blood shots in the cardinal directions. |
Molear | Burrows under the ground and follows Isaac. After a random period of time, or after the room is cleared, it will emerge and become vulnerable. |
Momma | Walks towards Isaac, dealing contact damage. Generally has three Babs that orbit it. If all of a Momma's babs are killed, it will open its eyes and pursue Isaac faster. |
Mud | Invulnerable enemy that chases Isaac, creating a slowing trail of creep wherever it goes. |
Mullinest | Moves around the room diagonally, occasionally spawning Flyders. On death, becomes a lvl2 Flyder. |
Pedestilder | Starts disguised as an item pedestal. When Isaac gets close, it will come to life, quickly leaping towards Isaac and dealing contact damage. |
Phantom | Chases Isaac, dealing contact damage. Upon being killed, has a 50% chance of respawning. |
Planetary Grimace | Stationary and invulnerable monster that creates a large, fast moving blood shot that orbits it. |
Plop | Randomly wanders around the room, firing clouds of brown poop shots in all directions. |
Possessed Fly | Invulnerable enemy that flies around the room diagonally. Has a shield that blocks Isaacs shots and grows in size when shot at, which slowly shrinks back to its normal size over time. Dies on room clear. |
Psymind | Floats away from Isaac, occasionally teleporting. Controls a large tear with four smaller tears orbiting it, that floats towards Isaac. |
Pud | Invulnerable enemy that chases Isaac, creating a slippery trail of creep wherever it goes. |
Pukie | Flies around the room diagonally, occasionally spitting out a Spit. Will sometimes shoot a puke shot instead. |
Pyron | Moves erratically through the room, occasionally leaving a fire on its trail. Enemies that are close to it become flaming. fire or explosive variants of themselves if they have. |
Raging Fly | Flies around the room diagonally, but will proceed to chase Isaac if he gets close. Its high speed causes it to miss quite often, circling Isaac in unpredictable patterns. Deals contact damage. |
Rock Fly | A high health attack fly that slowly hovers towards Isaac. |
Rolly Polly | Charges up, and then rolls at Isaac at high speeds, slightly curving towards his direction if he moves, and leaves a trail of damaging red creep where it rolls. |
Rubble | Stationary enemy that is harmless, so long as it is not shot or approached. If Isaac shoots it, gets too close, it will become a swarm of five Rock Flies. Dies on room clear. |
Sack Fly | Flies slowly towards Isaac, occasionally firing a cluster of shots towards Isaac. When killed, fires multiple short-ranged projectiles around itself. |
Scolexian | Hops out of the ground and burrows elsewhere, firing 4 blood shots in the ordinal directions. Occasionally fires four bursts of three-way rings of shots at Isaac, or a barrage of explosive shots. |
Screep | Crawls along walls to try to line up with Isaac and shoots three linear bursts of blood shots in random directions. |
Shoosh | Stationary enemy that occasionally releases a large spore cloud-like volley of shots that stay in place, slowly falling to the ground. Takes increased damage in the brief period after attacking. |
Skeletooth | Follows Isaac and periodically dashes towards him when it gets close. Deals contact damage. At 50% HP, it will enrage, firing three teeth at Isaac and chasing him at a much faster speed. |
Slyde | Walks towards Isaac, dealing contact damage. Leaves a trail of slippery creep. |
Smasher | Stalks Isaac, following him in loose circles. Travels faster and faster until it loses control and shoots off in the direction it was going, destroying any obstacles that get in the way. |
Smoker | Pops out of the ground in random locations and coughs up a smoke ball, if one is not present already. |
Soul Wisp | Groups up with other wisps in the room, and circles a random enemy, protecting it from shots. |
Stitches | Walks towards Isaac, dealing contact damage. When it gets close enough to Isaac it will teleport behind him, and try and attack then. |
Thrasher | Stalks Isaac, following him in loose circles. After a certain period of time, or if Isaac gets too close, it will proceed to charge at him three times in rapid succession. After the third charge, it will continue in the direction it was headed until it hits an obstacle or wall. |
Thumper | Stationary enemy that is normally invulnerable. Raises its head up when Isaac gets close, revealing a fleshy stem that can be damaged. Upon raising its head, it begin to shake, whereupon it will slam its head down, firing tears in all directions. |
Tinga | Slowly moves around the room, occasionally hopping to spawn Dank Chargers or fire 8 shots in all directions. Splits 2 Dank Squirts upon death. |
Transcendental Creep | Crawls along walls to try to line up with Isaac and shoots three blood shots in a line. Unlike traditional creep enemies, the tears will be shot behind it, and will loop around from the other side of the room and back towards this enemy. |
Triple Horf | Stationary enemy that fires three medium ranged blood shots at Isaac when he gets in range of them. |
Vissure | Randomly wanders around the room. Pauses to charge and fire a long trail of red creep when Isaac gets in its line of sight. |
Wallow | Quickly leaps towards Isaac, leaving a spray of tears where it goes, and deals touch damage. Occasionally stops to cry, spraying even more tears around the room. |
Waxhead | Walks around the room, avoiding Isaac, frequently shooting projectiles that leave flames where they land. It head melts while it's alive and, after it has completely molten, it falls off the body, becoming a flame, and it dies. |

Brainfrost | Swiftly slides through the room, leaving a trail of Icy slippery creep. When Killed, fires multiple short-ranged projectiles around itself. |
Brainlet | Chases the player, dealing contact damage. All tears and projectiles in the room are slightly moved in the direction of its movement, including Isaac's own tears. |
Carcass | Stationary monster that occasionally spawns a variety of flies. |
Clone | Chases Isaac, dealing a full heart of damage. Upon taking enough damage, it collapses into two piles of goo, which, if not killed, regenerate as 2 Clones, with reduced health. |
Conjoined Globin | Chases Isaac, dealing a full heart of contact damage. Upon taking enough damage, it collapses into two piles of goo, which, if not killed, will regenerate into Globins. |
Cyst | Hops around the room, occasionally shooting a six-way ring of shots. On death, spawns a variety of worm enemies. |
Deadeye | Walks towards Isaac, dealing contact damage. When it has a direct line of sight of Isaac, will fire a quick burst of four tears in his direction. |
Devote | Flees from the player. If it is left alone for too long, will convert itself into an Ascended. |
Dimple | Stationary enemy that decreases in size when hit, but regenerates to its full size until fully destroyed. Occasionally spawns Ring Flies when fully swollen. |
Drone | Invulnerable enemy that flies around the room diagonally, dealing touch damage. Has a shield around it that blocks Isaacs tears and grows when shot at. The shield shrinks in size over time. |
Eyebomination | Chases Isaac while firing in many directions unpredictably. |
Eyezor | Walks towards Isaac, dealing contact damage. When it has a direct line of sight of Isaac, will simultaneously fire three quick bursts of four tears in his direction, and a few degrees left and right. |
Fissure | Randomly wanders around the room. Pauses to charge and fire a cone of poisonous creep towards the player, alongside a volley of ipecac shots. |
Frozen Ghost | Large tanky enemy that slides around the room easy, creating a small trail of light blue, slippery creep. If killed, will spawn a Wizoob. Does not need to be killed to clear the room. |
Gazer | Would charge towards the player, accelerating until it hit a wall. If it reached max speed, it would spawn a creep trail. |
Glioma | Moves around the room diagonally, occasionally firing brimstone in an X or + pattern (signalized by its animation). |

Hungry | Slowly walks towards Isaac. Periodically, it speeds up in the direction it was going, leaving a trail of red creep while doing so. If it deals contact damage, it becomes a Blank |
Listener | Chases Isaac. If the player stands still and without shooting, they eventually stop until the player resumes moving or shoots again. |
Lockpick | Flees from Isaac. If not killed quickly, creates a hole in the ground and jumps into it, disappearing. On death, drops 3-5 keys. |
Ludovican | Chases the player, dealing contact damage. All tears and projectiles in the room are slightly moved in the direction the Ludovician is walking. |
Melanoma | Pops out of the ground in random locations and shoots 6 shots, before quickly popping up somewhere else. If not killed fast enough, becomes a Tumor |
Parasitic | Walks towards Isaac, dealing contact damage. Spawns a weakened Suture (from the Gemini boss) on death, which chases Isaac. |
Peptic Ulcer | Pops out of the ground in random locations and randomly spawns spits, or fires volleys of explosive shots. |
Possessed Screep | Crawls along walls to try to line up with Isaac and shoots a volley of purple fires in random directions. |
Red Toadie | Tries to hop away from Isaac to get at a moderate distance away from him. Will try and grab him, his attack flies, or any other flies in the room with its tongue, and then burp up a volley of shots. |
Rock Nest | Stationary enemy that occasionally spawns Rock Flies and shoots rock projectiles in random directions when hit. On death, it may create a shockwave around it. |
Scout | Attaches itself to a wall, following it. A few seconds after spawning, it starts shooting a continuous laser in the direction it is heading. |
Sebal | Stationary enemy that decreases in size when hit, but regenerates to its full size until fully destroyed. Fires multiple short-ranged brown projectiles in random directions when fully swollen. These projectiles leave a large amount of light brown slippery creep. |
Thirsty | Wanders aimlessly, but, if it gets line-of-sight with the player, it will give chase, only stopping if it breaks line of sight., |
Toadie | Tries to hop away from Isaac to get at a moderate distance away from him. Will try and grab him, his attack flies, or any other flies in the room with its tongue, and then burp up a poisonous cloud. |
Toxic Sludge | Slides around the room, occasionally stopping to spew a fountain of short-range sludge shots in all directions or to spit a line of slippery brown creep. Spawns a Growth upon death. |
Virus | Can only move via teleporting. Will try and predict the players movements and spawn 2 tiles away from where the player is heading. |
Volatile Host | Stationary enemy that is normally invulnerable. When Isaac gets close, its head will fly off, shooting it and a volley of tears towards Isaac, and leaves behind its fleshy stem, which leaves a pool of creep beneath it and fires blood shots randomly in all directions. |
Wandering Eye | Attaches itself to a wall, following it. A few seconds after spawning, it starts shooting a continuous laser in the direction it is heading. |
